Lower Eyelid Surgery – Restoring Balance Between the Eye and Cheek

The area beneath the eyes has a significant influence on how the entire face is perceived. Over time, the fat pads may become more prominent, the skin may lose elasticity, and the transition between the lower eyelid and the cheek can become more defined. This may create bags and shadows that make the face appear tired, even when you do not feel tired.

At AGATA, lower eyelid surgery is not simply about removing bags. The aim is to restore a soft and natural transition between the eyelid and cheek, while preserving the natural shape and support of the lower eyelid.

How is the procedure performed?

During the consultation, we assess skin elasticity, the fat pads, the tone of the lower eyelid, and the natural support provided by the cheek. The procedure is then tailored to the individual anatomy.

The operation can be performed under local anaesthesia or general anaesthesia and typically takes around 1–1½ hours.

In patients without significant excess skin, the fat pads can be treated from the inside of the eyelid, without making an incision in the skin. Depending on the anatomy, the fat may be removed, reduced or repositioned to create a smoother transition between the lower eyelid and cheek.

If there is also wrinkled or loose skin and reduced muscle tone, the incision is placed discreetly just below the eyelashes. Through this approach, the fat pads can be treated, excess skin removed, and the muscle lifted and stabilised.

In some patients, a canthopexy is also required, where the outer part of the lower eyelid is gently tightened and supported. This helps preserve the natural position and stability of the lower eyelid after surgery.

Recovery

After surgery, swelling, bruising, mild irritation and a temporary feeling of tightness around the eyes may occur. This is a natural part of the healing process.

The swelling gradually subsides over the following weeks, and the result becomes more apparent as the tissues settle. The final result is typically assessed after 3–6 months.

Are there risks?

All surgical procedures involve some degree of risk, but the great majority of patients have an uncomplicated recovery.

Swelling, bruising, temporary dryness or irritation, mild asymmetry during healing and changes in sensation may occur. Less commonly, there may be under- or overcorrection, a change in eyelid position, infection or a collection of blood.

A careful assessment of lower eyelid support and individual anatomy is therefore an important part of planning the procedure.

What can you expect from the result?

The aim is a more rested and harmonious appearance, with reduced bags and shadows and a softer transition between the lower eyelid and cheek.

The goal is not to change the shape of the eye, but to preserve its natural character and restore balance and lightness to the area around the eyes.
