Prominent Ears – When Position Creates Harmony

Prominent ears are not an uncommon anatomical variation. In children, they can sometimes become a reason for teasing or bullying. In both children and adults, they may also affect self-consciousness. Some people choose to hide their ears with their hair or avoid certain hairstyles.

At AGATA, correction of prominent ears is not about changing the character of the ear, but about bringing the ear closer to the head in a more natural and harmonious position.

During the consultation, we assess the shape and symmetry of the ears, the distance between the ear and the head, the quality and flexibility of the cartilage, and whether the prominence is caused by an underdeveloped fold, a deep conchal bowl, or a combination of these factors.

How is the procedure performed?

The procedure is usually performed under local anaesthesia in adults and in motivated children who are able to cooperate during the operation.

The incision is most often placed discreetly behind the ear. From here, the cartilage is reshaped as needed. This may involve creating or defining the natural fold of the ear, or reducing the depth of the conchal bowl. Permanent sutures are used to secure the ear in its new position.

In some cases, incisions may be required on both the front and back of the ear, for example when the conchal bowl also needs to be reduced.

The aim is not to place the ear completely flat against the head, but to create a natural and balanced angle that suits the overall proportions of the face.

The procedure typically takes 1–2 hours and is performed on an outpatient basis.

Recovery

A light compressive dressing is typically worn for around 1 week, followed by a headband worn day and night for a further approximately 2 weeks.

Normal daily activities can usually be resumed after around 24 hours, while sports and contact activities should be avoided for approximately 4–6 weeks.

Are there risks?

All surgical procedures involve some degree of risk, but otoplasty is generally associated with a high level of patient satisfaction.

Possible risks include minor oozing, a visible or thickened scar behind the ear, asymmetry, temporary tenderness, or altered sensation. In some cases, a minor recurrence may occur, with the ear gradually moving slightly outwards again.

What can you expect from the result?

Swelling and tenderness usually improve over the first 2 weeks, while the result continues to settle gradually over the following months. For most patients, the correction is long-lasting.

The aim is to preserve the natural shape of the ear while placing it in a more harmonious position, so that attention is drawn more naturally to the face as a whole.
